The tragedy brings the local road toll to seven so far this year, with the total across NSW increasing to 167, seven more than the same time last year.
The collision last Sunday about 9am blocked Milperra Road in both directions.
One of the drivers, a man in his 60s, was treated by paramedics but died at the scene, while a female front passenger was taken to hospital with minor injuries.
The driver of the other car, also a man in his 60s, was taken to hospital in a stable condition.
